The Irish Hospice Foundation are currently working on a 3 year programme called ‘Changing Minds’. The purpose of the project is to promote excellence in end of life care for people with dementia. As part of this programme , we are developing a suite of 7 guidance documents for staff supporting people with dementia in all settings. We are delighted to announce that our first guidance document "Facilitating discussions on future and end-of-life care with a person with dementia" and the accompanying factsheet are now available for download.They are available here for download. We also have a dementia contact database which people can sign up for by emailing me at sarah.cronin@hospicefoundation.ie
